1. Gather Required Information
Before starting, have the following details ready:
- Your Order Number (found on your order confirmation email or receipt)
- Approved Number (received in the “approved” message from CYOF)
- Product Information (description and size of the product you received)
Having this information at hand will speed up the form-filling process.
2. Fill in Customer Information
In this section, you’ll enter your personal details so CYOF can easily identify and contact you about your exchange request.
- Full Name: Enter your full name as used on the order.
- Order Number: Provide the unique order number associated with your purchase.
- Email Address: Use the same email that you used for the order.
- Contact Number: Enter a phone number for updates or if CYOF needs additional information.
3. Enter Approved Exchange Details
After receiving approval for your exchange, enter the specific details here:
- Approved Number: Copy the approved number exactly as received in the message from CYOF.
- Date of Approval: Note the date you received the approval.
4. Describe Original Product Details
This section requires information about the product you wish to exchange.
- Product Name/Description: Provide the name and description of the product as it appears in your order details.
- Product Size: Enter the original size of the product received.
- Reason for Exchange: Indicate whether the exchange is due to receiving an incorrect or damaged item.
5. Fill in Exchange Details
Here, you’ll specify the details of the product you’re requesting as a replacement.
- Replacement Product Name/Description: Enter the product name and description for the replacement.
- Preferred Size: If applicable, provide the size of the replacement product.
Note: CYOF does not offer exchanges for size or color changes, only for incorrect or damaged products.
6. Provide Return Information
After sending your item back, fill in the details of the return.
- Date of Return Submission: Write down the date you shipped the return.
- Tracking Number of Return Package: Include the tracking number provided by your shipping carrier.
7. Attach Required Files
For your exchange request to be approved, you need to submit specific attachments.
- Unboxing Video: Upload the video showing the unboxing of the product to verify its condition on arrival.
- Product Photographs: Include three clear photos:
- Photo 1: Showing the product in its original packaging with all tags visible.
- Photo 2: Close-up of tags or labels on the product.
- Photo 3: Clear image of the box with the “Exchange Label,” along with your signature and the date.
8. Confirm Exchange Label Photo
Take a photo of the signed and dated Exchange Label and upload it here. This label is placed on the same area as the original box label for easy identification.
9. Declaration
At the end of the form, you’ll make a declaration regarding the condition of the product being returned.
- Check the box confirming that the item is unused, unworn, unwashed, and in its original packaging with all tags intact.
- Sign and date the declaration to acknowledge your agreement with CYOF’s exchange policies.
10. Submit the Form
Once all fields are filled and files are uploaded, review your entries for accuracy. When ready, submit the form. CYOF will process your exchange within 7-10 working days from when the returned item is received.
